Thriving Mind South Florida is seeking a Care Coordinator for the Jail-In Reach Court Grant. This position will facilitate the transition from jail to community based mental health treatment and services. Assist with linkage to community based mental health treatment and services including needs assessment, discharge planning, housing, and initiation of application for entitlements. Will access other essential support services for the client following discharge to the community. Provides information to the court regarding the client

QUALIFICATIONSBachelor’s Degree in criminal justice, psychology, sociology, public health, or related fields and at least 3 years of practical experience in community mental health, forensic mental health, or criminal justice.

SKILLS & KNOWLEDGE REQUIRED:

  • Knowledge of issues related to criminal justice, community mental health, case management and community resources
  • Knowledge of personal computer software, i.e. Word, WordPerfect, Excel, Internet Explorer
  • Ability to communicate effectively orally and in writing
  • Ability to follow oral and written instructions
  • Detail-oriented, goal-oriented
  • Ability to maintain complex records
  • Ability to establish and maintain effective relationships with co-workers, the judiciary, court administration, and other agencies
